Gonadorelin (GnRH)
Gonadorelin is synthetic gonadotropin-releasing hormone (GnRH), a prescription decapeptide that stimulates the pituitary to release LH and FSH — used clinically for HPG-axis testing and pulsatile fertility therapy, and off-label as a TRT adjunct.
What Is Gonadorelin?
Gonadorelin is a synthetic decapeptide identical to natural gonadotropin-releasing hormone (GnRH), the hormone made by the hypothalamus that tells the pituitary gland to release luteinising hormone (LH) and follicle-stimulating hormone (FSH). Those two gonadotropins in turn drive the gonads — stimulating testosterone and sperm production in men, and the ovarian cycle in women. Because it is chemically the same as the body’s own signalling peptide, gonadorelin sits at the very top of the hypothalamic–pituitary–gonadal (HPG) axis. It is a peptide drug rather than a casual dietary supplement: it is manufactured, prescribed, and monitored in a clinical setting, and is also sold as gonadorelin acetate.
What It’s Used For
Gonadorelin’s established uses are clinical. As a diagnostic agent, a single dose is given and the resulting LH/FSH response is measured to test whether the pituitary–gonadal axis is working — the GnRH stimulation test. As a therapy, delivered in carefully timed pulses (usually via a programmable infusion pump), it can restore gonadotropin release and fertility in people whose own hypothalamus fails to produce GnRH properly (hypogonadotropic hypogonadism). Separately, gonadorelin has become popular off-label in TRT and bodybuilding circles, where it is used alongside exogenous testosterone in an attempt to keep the testes signalled — preserving testicular size and some fertility that suppressive TRT would otherwise shut down. That off-label use is real but sits outside the drug’s approved, evidence-backed indications; gonadorelin remains a prescription clinical agent.
How It Works
Gonadorelin binds GnRH receptors on the pituitary gland, prompting release of LH and FSH. The single most important feature of its pharmacology is that the pattern of dosing matters more than the amount. Delivered in brief, spaced pulses — mimicking the body’s own roughly 90-minute rhythm — it stimulates the axis and sustains gonadotropin output. Delivered continuously or at sustained high levels, it does the opposite: the pituitary receptors desensitise and downregulate, and LH/FSH release is suppressed. This stimulate-versus-suppress duality is exactly why long-acting GnRH agonists are used to shut down sex hormones in conditions like prostate cancer and endometriosis. A related quirk is an initial “flare,” where the first doses can transiently raise LH/FSH before any suppression sets in. Gonadorelin also has a very short half-life — on the order of minutes — which is what makes precise pulsatile delivery both necessary and possible.
Dosage
There is no consumer or oral dose for gonadorelin. It is an injectable, clinician-directed drug, and both the amount and the delivery pattern are set by a prescriber for a specific purpose. Diagnostic testing uses a single defined dose; fertility therapy uses small, repeated micro-doses delivered on a fixed pulsatile schedule (typically through an infusion pump) to imitate the body’s natural rhythm. Safety
