Picamilon

Picamilon is a synthetic conjugate of GABA and niacin developed in the USSR for anxiety and cerebral blood flow; the FDA has ruled it is not a lawful dietary ingredient.

What Is Picamilon?

Picamilon is a synthetic molecule made by joining GABA to niacin (nicotinic acid) — the niacin portion is intended to carry GABA across the blood–brain barrier, where the molecule is thought to release GABA. Developed in the Soviet Union, it is used there for anxiety and to improve cerebral circulation. In the US it is not a lawful dietary ingredient: the FDA ruled in 2015 that picamilon does not meet the statutory definition of a dietary ingredient and sent warning letters to companies selling it 1Reference 1U.S et al. · 2015Picamilon in dietary supplements — regulatory determinationView study →. Its efficacy evidence is old and largely Russian-language, and recent work casts doubt on whether the intact molecule is even biologically active.

Evidence

An early pharmacology report (“the new cerebrovascular preparation pikamilon”) described increased cerebral blood flow and cerebrovascular effects, which is the basis of picamilon’s “blood flow” reputation 2Reference 2Kruglikova-Lvova et al. · 1989AnimalThe new cerebrovascular preparation pikamilon — in vivoView study →. The mechanistic rationale is that the niacin conjugate ferries GABA across the blood–brain barrier, after which the molecule is hydrolysed back to GABA and niacin — but this is a proposed mechanism, not a demonstrated clinical effect. Importantly, a 2023 screening study found that picamilon itself was inactive against 50 biological targets, including GABA receptors, suggesting any real effect (if present) comes from downstream hydrolysis products rather than the parent compound 3Reference 3Santillo et al. · 2023In vitroPicamilon, a γ-aminobutyric acid (GABA) analogue and marketed nootropic, is inactive against 50 biological targets — in vitroView study →. Its widely marketed use as an anxiety/“calm-plus-focus” nootropic rests mainly on this old literature and mechanistic argument; I did not find strong modern human trials in this quick pass.

Dosage & Safety

Doses used in the former USSR were reportedly in the region of 50–200 mg/day; this describes historical use, not a personal recommendation, and rests on non-English clinical literature that is hard to verify at source. In the US, picamilon is not a lawful dietary supplement ingredient per the FDA determination 1Reference 1U.S et al. · 2015Picamilon in dietary supplements — regulatory determinationView study →, and products sold as such are considered misbranded. Given the niacin component, high doses could plausibly produce niacin-type effects (e.g. flushing), though this is a mechanistic note, not a measured finding. Long-term safety, drug interactions and use in pregnancy are not established. This entry is informational only. Toxicity has not been formally characterised here.
