Comfrey
Symphytum officinale
Comfrey (Symphytum officinale) — a topical-only 'knitbone' herb for bruises, sprains, closed fractures and blunt injury, rich in tissue-healing allantoin.
What Is Comfrey?
Comfrey is a big, bristly, deep-rooted perennial herb of the borage family, grown in gardens across the temperate world and known for centuries by the blunt, telling name knitbone. Its long reputation is as a first-aid herb for physical injury — bruises, sprains, strains and broken bones — applied to the skin over the hurt as a poultice, salve, oil or cream.
That reputation rests largely on one compound: allantoin, a substance found abundantly in comfrey’s root and leaf that encourages cells to divide and connective tissue, cartilage and bone to knit and repair. It is paired with soothing mucilage and anti-inflammatory constituents, which is why comfrey has always been reached for after a fall, a sprain or a set fracture.
One rule governs everything about comfrey: it is a topical-only herb. The same plant also contains pyrrolizidine alkaloids that are toxic to the liver, so — unlike its historical use — comfrey is no longer taken internally. It belongs in the first-aid cabinet as a cream or salve, not in the teapot.
What Is Comfrey Used For?
Comfrey is used externally to speed the healing of blunt physical injury and to ease the pain, swelling and stiffness that come with it. It is a classic component of injury salves, “bruise balms,” bone-and-muscle rubs and after-sport creams.

Traditional Uses
Comfrey’s country names tell its whole story — knitbone, boneset, bruisewort, blackwort. Across European folk medicine the root was grated or pounded into a wet paste and bound over set fractures and sprains, where it stiffened around the limb almost like a plaster cast while its allantoin drew the tissue back together. Poultices of the mucilaginous leaf were laid over bruises, swellings and inflamed joints.
Historically comfrey was also taken internally, as a tea or the eaten leaf, for coughs and digestive complaints — a use modern herbalism has abandoned since the discovery of its liver-toxic pyrrolizidine alkaloids. Today the tradition survives only in its external form, as the injury herb par excellence.
Botany
Comfrey (Symphytum officinale) is a robust, clump-forming herbaceous perennial of the borage family (Boraginaceae), reaching roughly 0.3–1.5 m tall, with branched, winged stems and the whole plant clothed in coarse, downward-pointing bristly hairs — the tell-tale roughness of its family. Its leaves are large, oval to lance-shaped and up to about 25 cm long, the lower ones markedly bigger. The flowers hang in the family’s characteristic coiled, drooping (scorpion-tail) clusters, each bloom tubular to narrowly bell-shaped and 8–20 mm long, in cream, yellowish, pink or purple, appearing through late spring and early summer.
Below ground it drives a thick, fleshy taproot — white inside but sheathed in a near-black skin — that descends well over a metre, letting comfrey mine deep subsoil nutrients and making an established plant very hard to remove. The root and leaf are the parts used, externally only.
A note on naming: much of the comfrey grown today is not the wild species but Russian comfrey (Symphytum × uplandicum), a vigorous hybrid of common comfrey with prickly comfrey (S. asperum), selected for bulk foliage — most named garden and compost cultivars belong to it.
Distribution
Common comfrey is native across temperate Europe into western Asia (east to western Siberia and northwestern Türkiye), where it grows in damp, grassy places — wet meadows, riverbanks, ditches and fen margins — favouring moist, fertile, lowland ground. It has been carried far beyond that range as a garden and medicinal plant and is now widely naturalized, notably across North America, persisting as an escape on roadsides, waste ground and streambanks. Because virtually any fragment of its root can regenerate a new plant, established stands spread and hold ground stubbornly, and it can be locally weedy — though it is not classed as a serious invasive.
Growing Conditions
- Grows in full sun to partial shade in moderately fertile, moist but reasonably free-draining soil; adaptable and tough once established.
- Extremely cold-hardy (to roughly −35 °C, about USDA zones 3–9) and drought-tolerant, thanks to its deep taproot.
- Propagated from root or crown cuttings — set pieces about 5 cm deep and ~60 cm apart; almost any root fragment left in the soil resprouts, so site it where you want it to stay, as it is difficult to dig out.
- A star of the productive garden: the foliage can be cut several times a season for compost, liquid plant feed or “chop-and-drop” mulch, regrowing quickly from the crown.
- Growers who want it to stay put usually choose the sterile ‘Bocking 14’ Russian-comfrey cultivar, which sets no seed and won’t sow itself around.
- Its nodding flower clusters are a magnet for bees and bumblebees, making it a useful pollinator plant in an orchard or kitchen garden.

Harvesting, Collection & Preparation
Leaves are gathered through the growing season; the root is dug in autumn when its stored compounds peak. Both are made up for external use only — the root and leaf infused into oil for salves and balms, pounded fresh into poultices, or worked into creams and liniments applied over the injured area. Because the root carries a much higher load of pyrrolizidine alkaloids than the leaf, many modern preparations favour the leaf or use PA-reduced extracts.
Phytochemistry
Comfrey’s activity is led by allantoin, a cell-proliferant that stimulates tissue growth and repair and underlies the plant’s “knitbone” reputation. It is joined by abundant mucilage (soothing and emollient on the skin), rosmarinic acid and related phenolics (anti-inflammatory), and tannins (mildly astringent), along with silicic acid, choline and asparagine. Set against these are the pyrrolizidine alkaloids — symphytine, echimidine, lycopsamine and others — the hepatotoxic constituents that confine comfrey to external use and that occur at much higher levels in the root than in the leaf.

Modern interest centres on topical comfrey extract for pain and blunt injury — creams and ointments for sprains, back pain and osteoarthritis have been the subject of clinical trials — with allantoin and rosmarinic acid as the plausible active principles.

Safety & Pregnancy
Comfrey is a topical-only herb — its pyrrolizidine alkaloids are toxic to the liver, so it must never be ingested, and even on the skin it is limited to intact skin, small areas and short periods.
- Never ingest. Hepatotoxic pyrrolizidine alkaloids can cause veno-occlusive liver disease; oral comfrey is restricted or banned in many countries.
- Intact skin only. PAs absorb through skin — use over limited areas for limited periods (a few weeks a year), never on broken skin or deep/dirty wounds.
- Avoid in pregnancy, lactation and liver disease. Also avoid in infants and children.
- Root carries more than leaf. The root has far higher PA levels — leaf-based or PA-reduced preparations are preferred.
- Can trap infection. Rapid surface cell-proliferation may close skin over an unhealed deeper wound, risking abscess — reserve for closed injuries and clean, superficial grazes.

Comfrey is a topical-only herb. It contains pyrrolizidine alkaloids (PAs) — symphytine, echimidine and related compounds — that are toxic to the liver and can cause hepatic veno-occlusive disease with internal use. It must never be taken internally (as tea, capsule or eaten leaf), and oral comfrey products are restricted or banned in many countries, including the US, UK, Germany, Canada and Australia.
Even applied externally, PAs can be absorbed through the skin, so topical comfrey is used on intact skin only, over limited areas, and for limited periods (commonly no more than a few weeks per year). It should not be applied to broken skin or to deep or dirty open wounds: comfrey’s rapid surface cell-proliferation can knit the outer skin closed before a deeper wound has healed from within, trapping infection and risking an abscess — so it is reserved for closed injuries and clean, superficial grazes.
Avoid comfrey entirely in pregnancy and breastfeeding, in liver disease, and in infants and children. The root carries far more PAs than the leaf, so leaf-based or PA-reduced preparations are preferred. As with arnica, it belongs in the first-aid kit as an external remedy, never as an internal one.

Avoid. Internal use is contraindicated (hepatotoxic PAs cross the placenta and enter breast milk), and topical use is also best avoided during pregnancy and lactation given absorption through the skin; do not apply to the breast area when nursing.
