Gastrodin
Gastrodin (4-hydroxybenzyl alcohol 4-O-β-D-glucoside) is the marker constituent of Gastrodia elata — tian ma, 天麻 — the classical Chinese anticonvulsant and anti-vertigo rhizome. It is largely a delivery form: in the gut and tissues it is cleaved to its aglycone 4-hydroxybenzyl alcohol, which crosses into the brain, so much 'gastrodin' pharmacology is really the aglycone at work. Unusually for a plant phenolic it has a real clinical footprint in China — an injectable gastrodin drug used for vertigo and vascular headache — but most of that human evidence is the injection or whole tian ma, and its neuroprotective and anticonvulsant mechanisms are largely preclinical.

Gastrodin at a Glance
Gastrodin, chemically 4-hydroxybenzyl alcohol 4-O-β-D-glucoside, is the headline constituent of Gastrodia elata — tian ma (天麻), the achlorophyllous medicinal orchid that is one of the most important nerve remedies in Chinese medicine. Tian ma is a classical treatment for headache, dizziness and vertigo, convulsions and childhood epilepsy, and gastrodin is the molecule used to identify and standardise it 1Reference 1ReviewGastrodia elata Bl.: A Comprehensive Review of Its Traditional Use, Botany, Phytochemistry, Pharmacology, and PharmacokineticsView study →.
The single most important thing to understand about gastrodin is that it is largely a delivery form. It is the glucoside of a smaller phenol, 4-hydroxybenzyl alcohol (HBA, also called gastrodigenin) — and in the gut and body that sugar is cleaved off by β-glucosidase, releasing the aglycone. Both the glucoside and the aglycone cross the blood–brain barrier, but the aglycone is the more lipophilic, brain-penetrant partner, so much of what is reported as “gastrodin” pharmacology is really 4-hydroxybenzyl alcohol at work 3,
Gastrodin is unusual among plant phenolics in having a genuine clinical footprint: in China an injectable gastrodin drug (“tian ma su”) has been used for decades for vertigo and vascular/tension headache, and it appears in modern Chinese randomised trials and meta-analyses 8,
Gastrodin, Parishin & p-Hydroxybenzyl Alcohol
The point of this molecule is the prodrug chain it sits in the middle of. Tian ma does not present a single active — it presents a small family of interconverting phenolics, and gastrodin is the hub that links the storage forms to the brain-active aglycone.
- Parishins (parishin A, B, C, E) are the upstream storage forms — gastrodin units esterified to citric acid. They are the most abundant actives in the fresh/aqueous rhizome, and they hydrolyse to gastrodin during steaming, drying, decoction and digestion. In rat distribution work, parishin behaves as a precursor pool that feeds gastrodin systemically 13Reference 13AnimalRelative tissue distribution and excretion studies of gastrodin and parishin from powder and extract of Gastrodiae Rhizoma in rat by UHPLC-ESI-MS/MSView study →. There is no parishin page here yet; treat it as gastrodin-in-waiting.
- Gastrodin is the glucoside — the stable, water-soluble, pharmacopoeial marker. It is what quality standards measure and what the injection delivers. But it is not very lipophilic, and its own tissue residence is short.
- 4-Hydroxybenzyl alcohol (HBA / gastrodigenin) is the aglycone — gastrodin minus its sugar. β-Glucosidase (in the gut microbiota and in tissue) cleaves gastrodin to HBA, which is more lipophilic and crosses into the brain, where much of the central activity is thought to occur 3,

Tellingly, the conversion is bidirectional — administered HBA can be re-glucosylated back to gastrodin in vivo — so the two circulate as an interconverting pair 4Reference 4AnimalPharmacokinetics of gastrodin and its metabolite p-hydroxybenzyl alcohol in rat blood, brain and bile by microdialysis coupled to LC-MS/MSView study →.
A third sibling, the aldehyde p-hydroxybenzaldehyde, sits alongside them in the rhizome as a minor marker with its own GABAergic profile. The practical consequence of the chain is a recurring interpretive trap: a study that doses gastrodin and sees a central effect may be reporting the action of HBA, and a study on whole tian ma is reporting the whole parishin → gastrodin → HBA system, not the glucoside alone.
Tian Ma & the Gastrodin Injection
Gastrodin is one of the few plant glucosides to have become a registered drug. In China, gastrodin injection (“tian ma su injection”) and oral gastrodin have been used clinically for decades, principally for two indications that map onto tian ma’s classical uses: vertigo/dizziness and vascular (and tension-type) headache 8,
- Vertigo and dizziness. A large retrospective cohort built from Chinese electronic health records analysed gastrodin injection in dizziness/vertigo and found it clinically used and reasonably tolerated; earlier single-blind randomised work reported it non-inferior to (and in symptom relief better than) betahistine injection, with a comparable low rate of adverse reactions 10Reference 10ObservationalClinical and economic analysis of Gastrodin injection for dizziness or vertigo: a retrospective cohort study based on electronic health records in ChinaView study →. This is real-world and trial human data — but it is the injectable drug, not an oral supplement.
- Migraine and headache. Gastrodin has been used for primary headache in China for more than thirty years. Meta-analyses pooling Chinese randomised trials report that gastrodin reduces migraine frequency, pain intensity and attack duration and slows elevated cerebral blood-flow velocity 8Reference 8Meta-analysisEffectiveness of gastrodin for migraine: A meta-analysisView study →, with a parallel meta-analysis in tension-type headache reaching similar conclusions 9Reference 9Meta-analysisYan Y, Zhang YL, Jin XL, Liao X, Gong X, Chen WJ, Wei JJ, Fan XM, Piao JZ, Fu GJ, Guo CL (2021). [Systematic review and Meta-analysis of efficacy and safety of gastrodin in treatment of tension-type headache]. Zhongguo Zhong Yao Za Zhi (China Journal of Chinese Materia Medica). https://pubmed.ncbi.nlm.nih.gov/34581069/View study →. Mechanistically, a 2024 mouse study tied gastrodin’s anti-migraine effect to inhibition of a succinate/HIF-1α/TRPM2 pathway in trigeminal-ganglion neurons 7Reference 7Gastrodin alleviates NTG-induced migraine-like pain via inhibiting succinate/HIF-1α/TRPM2 signaling pathway in trigeminal ganglionView study →.
The load-bearing caveat is quality and form. The favourable headache/vertigo meta-analyses rest largely on Chinese-language RCTs of variable methodological quality, frequently using the injection and frequently as an add-on to other treatment — so the effect size and the contribution of gastrodin alone are uncertain. The clearest test of oral isolated gastrodin is the ongoing multicentre, placebo-controlled EASTERN trial in medication-overuse headache, whose results are not yet in 11Reference 11RCTEfficacy and tolerability of oral gastrodin for medication overuse headache (EASTERN): Study protocol for a multicenter randomized double-blind placebo-controlled trialView study →. Until then, “tian ma works for headache and vertigo” is better supported than “oral gastrodin, on its own, is proven.”
What It's Used For
Gastrodin’s research is broad and coherent — it clusters tightly on the central nervous system — but it splits sharply by route and form: a real (if uneven) human signal for the injection/whole herb in headache and vertigo, sitting on top of a large preclinical mechanistic body for neuroprotection, epilepsy and cognition.
Support is an experimental score — weighted by study type (human > animal > in vitro) and volume. A beta, not a validated metric. The two headache/vertigo rows carry human trial data (mostly injection/whole-herb, variable quality); the CNS-mechanism rows are preclinical. Read the per-row basis.
The strongest human signal, and the reason gastrodin is a drug and not just a research phenol. Gastrodin has been used for primary headache in China for over thirty years, and a 2022 meta-analysis of 16 randomised trials (1,332 subjects) found it significantly reduced migraine frequency, pain degree and attack duration and slowed raised average cerebral blood-flow velocity 8Reference 8Meta-analysisEffectiveness of gastrodin for migraine: A meta-analysisView study →; a separate systematic review/meta-analysis reached comparable conclusions in tension-type headache 9Reference 9Meta-analysisYan Y, Zhang YL, Jin XL, Liao X, Gong X, Chen WJ, Wei JJ, Fan XM, Piao JZ, Fu GJ, Guo CL (2021). [Systematic review and Meta-analysis of efficacy and safety of gastrodin in treatment of tension-type headache]. Zhongguo Zhong Yao Za Zhi (China Journal of Chinese Materia Medica). https://pubmed.ncbi.nlm.nih.gov/34581069/View study →.
A 2024 mechanistic study gave the effect a plausible target: in nitroglycerin-induced migraine mice, gastrodin raised sensory thresholds by inhibiting a succinate/HIF-1α/TRPM2 cascade (and the downstream calcium overload) in trigeminal-ganglion neurons 7Reference 7Gastrodin alleviates NTG-induced migraine-like pain via inhibiting succinate/HIF-1α/TRPM2 signaling pathway in trigeminal ganglionView study →.
Gap: the pooled trials are largely Chinese-language, of variable methodological quality, frequently use the injection and frequently as an add-on — so the isolated oral effect is unproven, which is exactly what the ongoing placebo-controlled EASTERN oral-gastrodin trial is designed to test 11Reference 11RCTEfficacy and tolerability of oral gastrodin for medication overuse headache (EASTERN): Study protocol for a multicenter randomized double-blind placebo-controlled trialView study →.
Tian ma’s classical anti-dizziness use, carried into a registered drug. A large retrospective cohort drawn from Chinese electronic health records analysed gastrodin injection in dizziness/vertigo and found it clinically established and reasonably tolerated 10Reference 10ObservationalClinical and economic analysis of Gastrodin injection for dizziness or vertigo: a retrospective cohort study based on electronic health records in ChinaView study →; the same body of work references single-blind randomised data in which gastrodin injection relieved vertigo at least as well as betahistine, with a comparable, low adverse-reaction rate 10Reference 10ObservationalClinical and economic analysis of Gastrodin injection for dizziness or vertigo: a retrospective cohort study based on electronic health records in ChinaView study →.
Gap: this is the injectable pharmaceutical used in Chinese clinical practice, evaluated mostly by cohort and modest RCTs — not a blinded, Western-regulatory trial of an oral isolate, and not generalisable to a self-dosed supplement 10Reference 10ObservationalClinical and economic analysis of Gastrodin injection for dizziness or vertigo: a retrospective cohort study based on electronic health records in ChinaView study →.
The mechanistic heart of tian ma’s traditional reputation. In seizure-sensitive gerbils, gastrodin decreased the immunoreactivity of the GABA-shunt degradative enzymes (GABA-transaminase, SSADH, SSAR) in the hippocampus and lowered seizure scores — consistent with raising brain GABA by slowing its breakdown 5Reference 5Gastrodin decreases immunoreactivities of gamma-aminobutyric acid shunt enzymes in the hippocampus of seizure-sensitive gerbilsView study →.
Reviews place this GABAergic, antioxidant and neuronal-excitability-stabilising action at the centre of gastrodin’s anti-epileptic and Tourette/tic research 2Reference 2ReviewA Review on Central Nervous System Effects of GastrodinView study →.
Gap: the anticonvulsant data are rodent (with the sibling p-hydroxybenzaldehyde carrying much of the direct GABA-transaminase inhibition), the effect is substantially mediated by the aglycone HBA, and there is no robust human trial of gastrodin as an antiepileptic — it remains an adjunct/traditional use, not a stand-alone anticonvulsant drug 2,
The largest preclinical literature. Across rodent and cell models of cerebral ischaemia/reperfusion, Parkinson’s, Alzheimer’s, vascular dementia and traumatic brain injury, gastrodin is repeatedly neuroprotective — lowering oxidative stress (↑ SOD/Nrf2 defences, ↓ ROS/MDA), damping neuroinflammation (NF-κB, microglial activation, NLRP3-type signalling), preserving mitochondria and improving learning/memory read-outs 2,
Gap: it is an entirely preclinical signal — animal and in-vitro models, not the human diseases — capped by gastrodin’s own poor tissue residence and the fact that much of the delivered activity is really the aglycone; no clinical neuroprotection outcome rests on isolated gastrodin 2,
A focused affective-behaviour signal. In rodent models, Gastrodia elata and its compounds gastrodin and 4-hydroxybenzyl alcohol produced antidepressant-like effects, acting through the monoaminergic system (serotonin/noradrenaline) and neuronal cytoskeletal remodelling 6Reference 6Anti-depressant effects of Gastrodia elata Blume and its compounds gastrodin and 4-hydroxybenzyl alcohol, via the monoaminergic system and neuronal cytoskeletal remodelingView study →; tian ma phenolics are also anxiolytic in mice, though there the sibling constituents carry much of the receptor-level action.
Gap: the affective data are rodent behavioural studies of the isolate(s), tie the effect to gastrodin plus HBA together rather than gastrodin alone, and have no human anxiety/depression trial behind them 6Reference 6Anti-depressant effects of Gastrodia elata Blume and its compounds gastrodin and 4-hydroxybenzyl alcohol, via the monoaminergic system and neuronal cytoskeletal remodelingView study →.
Mechanisms
| Target / pathway | Effect | Relevant to |
|---|---|---|
| β-Glucosidase (gut microbiota + tissue) | deglycosylates gastrodin → 4-hydroxybenzyl alcohol, the brain-penetrant aglycone; interconversion is bidirectional | the whole pharmacology (prodrug activation) |
| GABA-shunt enzymes (GABA-transaminase, SSADH, SSAR) | ↓ immunoreactivity → slows GABA breakdown → raises brain GABA | anticonvulsantsedative |
| Succinate / HIF-1α / TRPM2 (trigeminal ganglion) | ↓ succinate accumulation and TRPM2-mediated Ca²⁺ influx → ↓ migraine-like pain | migraine / analgesia |
| Oxidative stress (Nrf2, SOD ↑; ROS, MDA ↓) | antioxidant; restores endogenous defences | neuroprotection |
| Neuroinflammation (NF-κB, microglial activation, NLRP3) | suppresses pro-inflammatory signalling | neuroprotection |
| Monoamines (5-HT / noradrenaline) + neuronal cytoskeleton | antidepressant-like remodelling (with HBA) | affective |
| Cerebral vasculature / blood-flow velocity | normalises raised cerebral blood-flow velocity | migrainevertigo |
The load-bearing point: gastrodin is best read as a water-soluble prodrug. Its defining pharmacokinetic step — cleavage to the lipophilic aglycone 4-hydroxybenzyl alcohol — is upstream of most of the CNS targets in this table, which is why studies dosing “gastrodin” and studies dosing whole tian ma often report the same effects 3,
Pharmacokinetics
Gastrodin is highly water-soluble and rapidly absorbed after oral or parenteral dosing, but it is not a durable circulating molecule: it is quickly deglycosylated to 4-hydroxybenzyl alcohol by β-glucosidase in the gut microbiota and tissues, and it has a short half-life 3,
Clinical Trials
Human evidence is real but form-skewed: it belongs mostly to the Chinese gastrodin injection and to whole tian ma, in headache and vertigo, with the isolated oral question still open.

Isolate vs. Plant Studies
The split is unusually clean for a plant molecule. The human evidence is closer to the plant/whole-drug end. Even where a trial names “gastrodin,” it is usually the injection or an extract standardised to gastrodin, used in Chinese practice for headache or vertigo — so the clinical signal belongs to tian ma delivered as gastrodin, not to a purified oral supplement 8,
The mechanistic depth is the isolate — but in animals. GABA-shunt suppression 5Reference 5Gastrodin decreases immunoreactivities of gamma-aminobutyric acid shunt enzymes in the hippocampus of seizure-sensitive gerbilsView study →, antioxidant/anti-neuroinflammatory neuroprotection 2,
Its Role in Plants
Gastrodia elata is a striking plant: a leafless, rootless, chlorophyll-free orchid that cannot photosynthesise and lives by parasitising the fungi Armillaria and Mycena for carbon. Its underground tuber is the drug tian ma. In that tissue, gastrodin and its relatives are phenolic products of the shikimate/phenylpropanoid economy, stored in large part as the citric-acid-ester parishins; gastrodin and free 4-hydroxybenzyl alcohol are released from these precursors, and a substantial share of the gastrodin found in a processed rhizome is generated during the traditional steaming and drying rather than stored as the free glucoside in the living tuber 1,
These are plant- and processing-biochemistry roles and should not be read as human pharmacology — the phytochemistry explains why tian ma must be prepared to be potent, not what the molecule does as a medicine.
Prevalence in Nature
Gastrodin is essentially a single-source molecule. Unlike most phenolics — which turn up in trace amounts across many plants — gastrodin is, for practical purposes, the marker of Gastrodia elata and is not a meaningful constituent of common food or medicinal plants. Within tian ma it is the dominant standardised active, quantified in every quality survey 1,
Major Natural Sources
| Source | Plant part | Reported content |
|---|---|---|
| Gastrodia (Gastrodia elata, tian ma) | dried rhizome | ~0.35–1.34% dry weight (wide origin/processing variation) 12Reference 12Quality Evaluation of Gastrodia Elata Tubers Based on HPLC Fingerprint Analyses and Quantitative Analysis of Multi-Components by Single MarkerView study → |
Only Gastrodia is a quantitatively meaningful source; occasional trace reports in other orchids are qualitative and tracked for a later pass.
Discovery & Synthesis
Gastrodin’s modern story is a Chinese-pharmaceutical one. Its identity was established when Chinese researchers isolated and characterised it from Gastrodia elata in the late 1970s, naming the glucoside “tian ma su” and pinning tian ma’s central activity to it; a total synthesis followed shortly after, and because the glucoside is straightforward to make, gastrodin is now manufactured synthetically at scale rather than extracted, supplying the injectable and oral gastrodin products used across China 1,
Patents: not yet researched.
Toxicity & Safety
Gastrodin carries a low flag. It is a well-tolerated, water-soluble phenolic glucoside with decades of clinical use in China as an injectable and oral drug, and its preclinical toxicology is reassuring: acute studies report no mortality or overt toxicity in mice at oral or intravenous doses up to ~5,000 mg/kg, and repeat-dose studies in mice and dogs showed no adverse histopathology at doses far above the therapeutic range 2,
The honest limits are those of a molecule whose human safety data come from Chinese clinical practice rather than Western trials. Post-marketing reports of the injection describe occasional adverse reactions — most often skin (rash), gastrointestinal upset and nervous-system complaints such as dizziness, numbness, fatigue or transient memory changes — generally mild and reversible; injectable use also carries the usual small risk of hypersensitivity reactions 10Reference 10ObservationalClinical and economic analysis of Gastrodin injection for dizziness or vertigo: a retrospective cohort study based on electronic health records in ChinaView study →. Because gastrodin’s central action runs partly through GABAergic and monoaminergic pathways, a theoretical additive effect with sedatives, anticonvulsants or other CNS-active drugs is worth flagging at medicinal doses, though it is not quantified. As always, the exposure that matters clinically is the registered drug or whole tian ma preparation, not a purified dietary isolate.
- Low intrinsic toxicity. No mortality in mice up to ~5,000 mg/kg; non-genotoxic whole-herb extract; clean 28-day oral toxicity — consistent with decades of clinical use 2,

- Injection-related reactions. Post-marketing reports note occasional rash, GI upset and nervous-system complaints (dizziness, numbness, fatigue), plus the usual small hypersensitivity risk of any injectable 10Reference 10ObservationalClinical and economic analysis of Gastrodin injection for dizziness or vertigo: a retrospective cohort study based on electronic health records in ChinaView study →.
- CNS-drug interactions untested. A theoretical additive effect with sedatives/anticonvulsants/monoaminergic drugs at medicinal — not dietary — quantities; no formal human interaction data.
Pregnancy/lactation is a per-herb / per-supplement decision and is not adjudicated on this molecule page; no dedicated reproductive dataset exists for the isolate, and no verdict is issued here.
Dosage
There is no independent Western supplement dose established for isolated gastrodin. Its real dosing is Chinese-pharmaceutical, and every figure below is a research or registered-drug dose, not a personal recommendation:
